Meet Absence Form

Use this form to let the coaches know which GCSL meets your child/children will be missing. If your child/children are going to miss a COSA meet, just don't fill out any COSA event cards for that meet.

Directions:

1. Fill in a parent name and phone number
2. Fill in the child/children's name(s) and age(s)
3. Click on the meets your child/children will be missing
4. Click the Submit button
