Click on the following links to access the various forms used for Swim/Dive team administration.
| Banquet RSVP Form | Use this form to RSVP to the banquet at the end of the season. |
| Emergency Medical Form | This form lists emergency contact information and any special medical needs of your children. All parents must fill out one of these forms each year. The forms can be returned to the mailbox folder labeled "Completed Emergency Response Forms". Hint: The link takes you to a "typeable" PDF. Type in the necessary information and save the PDF form to your computer's hard drive. Next year you'll have a completely filled in form ready to sign and turn in. |
| Meet Absence Form | Use this form to notify the coaches that your swimmer or diver will miss an upcoming meet. |
| Volunteer Sign-Up Form | Use this form to sign up for volunteering for a swim meet. You don't have to visit the pool to sign up, just click on the link. |
| Otter Apparel Form | Use this form to order Otter apparel. It is due back no later than June 10. |
